Telehealth & Clinical Visits
Telehealth represents the electronic exchange of health information from one location to another; which includes images, vital signs, or other health related data regarding patient status. Health education, patient monitoring, wireless applications and call centers, are all part of the StratiHealth Telehealth program.
Business Case for Telehealth
Reducing the cost of health care, improving efficiencies and more importantly maintaining and ensuring wellness of a stable patient and early identification of issues that lead to an unstable patient are the goals of telehealth.
The StratiHealth expertise supports you gaining the edge on overcoming the challenges facing health care providers and organizations through telehealth.
Operational efficiency and improved outcomes are gained through the use of technology by creating solutions that improve patient engagement and care.
We provide initial consultation with a network of physicians, nurse practitioners and respiratory therapists in coordination with the patient's primary care physician, to risk stratify patients and establish a plan of care unique to the specific patient as a component of our Respiratory Patient Centered Care™ (RPCC™) program for Chronic Obstructive Pulmonary Disease (COPD), and establish a management and monitoring plan of care for patients with Congestive Heart Failure (CHF), Diabetes, and Chronic Non-Healing Wounds.
Our services and management support patients following discharge from an acute care hospital and those who require long-term treatment and management with medication, technology and oversight in their place of residence and other levels of care.
A skilled team of physicians, nurse practitioners and respiratory therapists are available to provide tele-consultative care any time of the day or night. Any member of the patient's care team—nurse, provider or respiratory therapist—can activate a consultation for any reason. In addition to managing post-acute care and chronic care management for patients with varying needs, mid-level providers (nurse practitioners) periodically visit patients, create care plans, address issues and challenges. Telehealth, treatment clinics and home visits allows our patients to receive comprehensive services from specialists closer to home for short-term or long-term management and support.
