We provide initial consultation with a network of physicians, nurse practitioners and respiratory therapists in coordination with the patient’s primary care physician, to risk stratify patients and establish a plan of care unique to the specific patient as a component of our Respiratory Patient Centered Care™ (RPCC™) program for Chronic Obstructive Pulmonary Disease (COPD), and establish a management and monitoring plan of care for patients with Congestive Heart Failure (CHF), Diabetes, and Chronic Non-Healing Wounds.

A skilled team of physicians, nurse practitioners and respiratory therapists are available to provide tele-consultative care any time of the day or night. Any member of the patient’s care team—nurse, provider or respiratory therapist—can activate a consultation for any reason. In addition to managing post-acute care and chronic care management for patients with varying needs, mid-level providers (nurse practitioners) periodically visit patients, create care plans, address issues and challenges.
