We provide initial consultation with a network of physicians, nurse practitioners and respiratory therapists in coordination with the patient’s primary care physician, to risk stratify patients and establish a plan of care unique to the specific patient as a component of our Respiratory Patient Centered Care™ (RPCC™) program for Chronic Obstructive Pulmonary Disease (COPD), and establish a management and monitoring plan of care for patients with Congestive Heart Failure (CHF), Diabetes, and Chronic Non-Healing Wounds.

Our services and management support patients following discharge from an acute care hospital and those who require long-term treatment and management with medication, technology and oversight in their place of residence and other levels of care.

A skilled team of physicians, nurse practitioners and respiratory therapists are available to provide tele-consultative care any time of the day or night. Any member of the patient’s care team—nurse, provider or respiratory therapist—can activate a consultation for any reason. In addition to managing post-acute care and chronic care management for patients with varying needs, mid-level providers (nurse practitioners) periodically visit patients, create care plans, address issues and challenges. Telehealth, treatment clinics and home visits allows our patients to receive comprehensive services from specialists closer to home for short-term or long-term management and support.
